There are a few reasons people live like that and you mentioned two: age and mental illness. Another, is drug use. Any place in that condition could be toxic if it were due to drug use/manufacture. I would be very cautious.

I`ve seen much worse than this. There are lots of people who`ve bought cheap homes to rent out that are in for an education.

this is probably a mental case, as stated, but not everyone has an excuse.
most people have no idea how the other half lives, that has no excuse. you have to buy them trash service because they see nothing wrong with letting trash pile up their (your rental) yard. brand new appliances go bad because they don’t clean them (think lard drippings on electrical) and as a slumlord to the government (section 8) you gotta replace it. if they forget their keys…no problem! they will just break a window to get in. they hate you because they owe you money. hate you as in want to kill you.

I assume this was a multi-unit property of some sort. The question is-what sort? Is that even a legal unit? I’ve seen bathrooms larger than that entire apartment.
